The spine at the same time loses its main function of the supporting column.

Fortunately, these complications are not too often. Most often, a clearly expressed picture of the disease is encountered with persistent chronic pain, caused even by the infringement of the root, but by constant muscle spasm. The reason is again discogenic - the vertebra is literally tired, overloaded, the disk is simplified, it does not adequately absorb loads, vibration, etc. This causes pain, at first, maybe small, but inevitably increases muscle tone.

The body tries to protect itself from pain with a “muscular corset”. But from constant muscle spasm is exhausted, its strength drops, the blood supply to the intervertebral disk (it goes through the muscles too) is deteriorating - here you have a vicious circle!

And now we, pardon the involuntary pun, we will return to “our sheep” - to the very one on whose example we studied the structure of the spine. Fortunately for them, our smaller brothers are free from the troubles I just talked about. Animals do not suffer from osteochondrosis, so it is difficult to study this disease in the experiment.

It is possible, however, to model the disease on rabbits or the same rams, but only if forcibly forcing the poor martyrs of science to stand in a special machine in an upright position for quite a long time.

No sooner had man got up from all fours and proudly straightened up, as nature immediately snapped him on the nose, or rather, on the back. In vain, some people are trying to argue that, they say, they need to eat less and move more. Having no objections on the merits of this thesis, let me turn to the facts. Paleontologists who have investigated the remains of our ancient ancestors have found signs of osteochondrosis from the very beginning Homo erectus - man erect (standing upright) - and it happened several hundred thousand years ago. And it is hard to suspect semi-monkey semi-humans to be in sybaritism. Thus, it can be argued that the osteochondrosis of the spine is a payment for walking on two legs.

Speaking of legs. In addition to the above described S-shaped spring, our body is also depreciated due to the foot. It has a longitudinal and transverse arches. The flattening of these arches is flatfoot. I will talk more about this later in a separate chapter.

Examining the patient, I definitely look at his feet. Too often, even the most slender legs are characterized by treacherous “bones” in the area of ​​the thumbs. I carefully look at my back - there will surely be signs of osteochondrosis!

What started earlier - flattening the arches of the foot or changes in the intervertebral discs?

Most likely, there was both. The so-called functional weakness of the ligamentous apparatus is quite widespread. People with this symptom often have subluxations of joints, sprains, minor injuries. And the reason is that the ligaments between the vertebrae are weak, and, among other things, the ligaments of the arches of the feet are weak. And if the physical activity is inadequate - that is, after long periods of passivity, a person begins to move a lot, walk for a long time or stand, lift unbearable weights, then the arches of the foot “collapse” and the spinal column can “look at”.

In this case, the specialist will be able to help, armed not only with knowledge and experience, but also with a certain “repair kit”: corsets for the spine, orthopedic insoles for shoes (below I will tell about these devices in more detail). All these tools, together with timely treatment will help reduce the unpleasant manifestations of osteochondrosis to a minimum.

But even in advanced cases, the position of the patient is not hopeless. It is possible, if not completely cure a person, then to significantly alleviate his condition - to restore working capacity, to allow one to live without pain and limited mobility.

And now let's talk more about the types of the disease. If we continue the comparison with the animal world, osteochondrosis is a multi-headed dragon. The disease is one, and she has a lot of varieties. The most common forms are cervical, thoracic and lumbosacral osteochondrosis.

In the East, the neck enjoys a special honor. It is believed that it connects the flesh and mind. Malfunctions in the cervical vertebrae create disharmony between the spirit and the body. Unfortunately, these symptoms often occur in women after 40 years, or even earlier.

Try a little test: is everything okay with your neck?

Slowly, relaxing the muscles of the neck, turn your head 90 degrees. If it is difficult for you to do this, or you feel a characteristic crunching when moving your head, then it is time for you to consult a doctor to check the condition of the intervertebral discs.

And now again, recall the anatomy of our skeleton.

If we look at the spine from the top down, then the cervical region comes first.

It consists of seven vertebrae, the first of which is called the atlas. Like the mythical hero, he holds the “sphere” of his head. Motionlessly fused with the skull, it is not like the other vertebrae, it does not have a cylindrical body and an intervertebral disk, as in the lower brethren.

The second vertebra, the epistrophy, is also not quite similar to the others. It has an prominent upward appendix, called the tooth, on which the atlas is planted and through which the atlas is rotated, as if on a vertical axis.

Further, the vertebrae gradually become more similar to each other - a cylindrical body, an annular arch, which forms the bed of the spinal cord, articular processes connecting with the upper and lower vertebra, processes for attaching muscles ...

The reasons for which may develop cervical osteochondrosis, a lot. These are various injuries of intervertebral joints and discs, disorders of the immune system, arthritis, meningitis, tuberculosis, and various tumors.

Incidentally, the lesion of intervertebral joints is the main cause of pain in the neck. Microdamages of these joints occur at any age, frequent microtraumas in old age lead to osteoarthritis.

A serious neck injury occurs during car accidents. When struck from behind, there is a sudden over-bending of the neck followed by a sharp bending of it, in a frontal collision a sudden bending followed by an extension.

With such an injury, muscles and ligaments, spinal roots, sympathetic trunk, intervertebral joints and discs are damaged. Intervertebral joints are particularly affected, even their microfractures (imperceptible on radiographs) are possible.

From my trauma practice I remember such a case.

The ambulance delivered a driver to our reception room, into whose car a truck ran into the back. The victim had a deep tetraplegia, that is, paralysis of the arms and legs, which occurs with fractures of the cervical vertebrae and squeezing of the spinal cord with bone fragments.

What was the amazement of the entire duty crew when we didn’t find the slightest bone fracture on radiographs!

Apparently, at the moment of the strike, this ill-fated “whip” worked: the vertebrae dislocated, then instantly set themselves right, but the cervical spinal cord was seriously injured. Unfortunately, subsequently the paralysis of the limbs of this person did not go through ...

Such injuries are usually accompanied by pain and limitation of head movements.

The pain is localized in the neck and shoulders, can give in the back of the head, in the interscapular region and in the hands. Symptoms usually appear in the first six hours after injury, sometimes during the day.

It is difficult to bend, and then throw back the head, there is a headache that can last for months. It usually hurts the back of the head, but can give to the temple and the orbit. Dizzy, nauseous.

Damage to the nerve roots during an injury or their compression of a herniated intervertebral disk is accompanied by radicular pain.

I am often asked: why precisely when cervical osteochondrosis often a headache, it gets dark in the eyes, the pressure rises (as in a patient I told you about)?

There is one feature characteristic only of the cervical vertebrae and cervical osteochondrosis. Important arteries, the vertebral and carotid, that supply blood to the brain, pass through the neck.

Carotid artery - the main of those that feed the brain. The carotid artery accounts for up to 80 percent of the blood flow.

In addition to the canal for the spinal cord, a canal passes here through the transverse processes of the vertebrae. vertebral artery, which goes into the cavity of the skull, feeding the cerebellum, the vestibular apparatus and the centers lying in the region of the neck and the base of the brain.

Artery "strung" on the vertebrae, like a string, a bunch of bagels. Only the hole in such a “donut” is very narrow, and the slightest displacement of the cervical vertebra leads first to reflex spasm of the vertebral artery, and in advanced cases, to squeezing it.

And add here a network of lymph nodes and blood vessels, nerve roots and trunks, through which communication with the hands, heart, lungs. In addition, in the neck is the thyroid and parathyroid glands, the upper part of the trachea, pharynx, the beginning of the esophagus.

In a word, any disturbance in the intervertebral discs of the neck causes various pathologies: headache, tinnitus, insomnia, pain in the shoulders ...

Therefore, the so-called vegetovascular insufficiency syndrome is characteristic of cervical osteochondrosis - the favorite diagnosis of local physicians when the “picture of the disease is unclear”. And the thing is that the blood supply to the equilibrium centers and a number of vegetative centers (that is, the managers of simple vital processes) is impaired.

To answer this question, go through one more. test:

Normalization of the state of the cervical spine leads to normal and blood pressure.

Practically everyone in the neck can find a small subluxation of one or several vertebrae. In addition, cervical osteochondrosis is present in many patients with radiculitis of various localization as a “free application”. And as a result - its complications.

There is a mysterious and unpleasant disease - one of those that hides behind osteochondrosis, as it were - Dupuytren's contracture.

And the cause of this disease is the same cervical osteochondrosis.

The cause of Dupuytren's contracture is a disorder of the trophism (tissue feeding process) due to impaired nerve fiber passage of the nerve. The nerve root, which forms one of the nerves of the hand, can be compressed by a displaced vertebra. Since it is known exactly which nerve from which vertebra of the cervical spine comes out, the numbness of one finger or another can be used to judge the displacement of a particular vertebra.

They may ask: why not all patients with cervical osteochondrosis develop similar contracture?

My answer: the manifestations of the same disease in each person may be different.

It has been observed that such wrinkling of the tendon cord is often observed in diabetic patients. In diabetes, trophic disorders that I mentioned are exacerbated.

There is an assumption that contracture is hereditary. You can put forward a lot of different versions of the causes of a particular set of symptoms. But undoubtedly the main thing - the main cause of the Dupuytren's contracture is a violation of the peripheral nerve conduction caused by the displacement of the cervical vertebrae.

In case of cervical osteochondrosis, radicular symptoms are often present - cervical radiculitis, when the pain is given to the arm, sometimes to individual fingers.Sometimes they are numb, cold, "marble" color.

By the way, the heroes of O. Henry, the thief and his potential victim, can be diagnosed with cervical osteochondrosis complicated by shoulder-blade periarthritis. The same symptoms, depending on the level of cervical osteochondrosis, occur in the elbow joint.

In the United States, this complication is called “frozen shoulder” and “frozen elbow.”

Thoracic osteochondrosis - a disease that is insidious precisely because of its similarity with other ailments. The reason is in its specificity: the thoracic region is inactive, the ribs, together with the spine, form a sort of carcass. This means that there are no such painful pains during movement and load, as in case of cervical or lumbar osteochondrosis, and therefore the symptoms similar to angina or myocardial infarction or respiratory function are in the foreground.

I will tell about one case from the practice.

My country house neighbor is a middle-aged man, an employee of a bank, his job is mostly sedentary. He comes to the cottage, and here he has a sharp change of activity: he is digging, building a house, carrying buckets of water ... This time he undertook to help the craftsmen to finish building the terrace. He lifted a heavy beam and turned, keeping it on weight. After that, he gasped and nearly fell - well, it was good that they managed to catch the beam ...

My neighbor was hospitalized to the regional hospital directly from the backyard with a diagnosis on admission - myocardial infarction.

His wife came to me the next day for a consultation. And it turned out that her husband often complained of pain in the interscapular region. Let's go with her to his hospital.

They gave me a look at the history of his illness, and this is what I read there: “The patient is pale, cannot inhale with full breastfeeding. Intense pain in the heart, not relieved by vasodilators. There are no signs of a heart attack on an ECG, but there are some changes characteristic of a malfunction of the heart muscle ... ”(which is sometimes observed during the first hours of a heart attack).

Pain associated with thoracic osteochondrosis of the spine are prolonged. The spread (irradiation) of pain in this case most often occurs according to the type of intercostal neuralgia - “gives up under the scapula”. This is where the patient has a fear of heart attack. The difference between ischemic heart disease and intercostal neuralgia is that when a patient has osteochondrosis, the patient has a normal (or almost normal) electrocardiogram and there are no such "heart" symptoms as abrupt pallor, cyanosis, salinity of the skin, cold sweat, etc.

The old doctors isolated another symptom that absolutely proved that the patient did not have a heart attack. - no fear of death. In addition, palpation of the corresponding vertebrae determines the local pain in the area of ​​the nerve passage.

Therefore, if the pain from vasodilating drugs (validol, nitroglycerin) does not decrease, and the cardiogram is close to normal, you need to examine the spine.

Pneumonia can also cause chest pain, but, unlike breast osteochondrosis, such pain does not have a clear location, pulmonary symptoms come to the fore: cough, fever ... Thoracic osteochondrosis can also cause pain in the right intercostal space. The patient is treated for cholecystitis, and he needs to correct the spine in the thoracic region.

You should be alerted if you feel:

• pain with a deep breath and exhale,

In terms of its clinical manifestations, intercostal neuralgia either mimics heart attack, angina pectoris, and similar heart diseases, like my neighbor banker, or masks severe disorders of internal organs, like another one of my patients whose history I want to bring here.

A young man came to me at the reception. He complained of acute pain in the chest: could not take a deep breath, cough. Appearance of the patient spoke about the deep troubles with health: pallor with jaundice tinge, glistening with sweat skin, hollow eyes. Doctors suspected a serious metabolic disorder ...

I found in the patient a displacement of one of the thoracic vertebrae. This led to intercostal neuralgia pain. The vertebra right, the pain gradually passed. But, unfortunately, the sufferings of the young man did not end there.

Upon further examination, an abscess (abscess) was found in the lung - what used to be called transient phpotis! Severe pain caused by displacement of the vertebra and intercostal neuralgia caused restriction of the respiratory movements of the chest, lung tissue did not smooth out on inspiration, possibly the bronchus was blocked, and as a result focal pneumonia developed with subsequent development of the ulcer. And this is the guy, it would seem, a healthy and living a full life! The patient received a special course of treatment of the lungs and after a while he fully recovered ...

Often, in thoracic osteochondrosis, intercostal neuralgia is accompanied by so-called “shingles” - blister cutaneous fever, the causative agent of which is the herpes virus. The stinging radiculitis pains are joined by burning and painful itching from bursting and weeping blisters of “fever”. Sometimes the temperature rises due to poisoning of the body with viral toxins.

Why does herpes affect the neuralgia zone and why most often in thoracic osteochondrosis?

The fact is that when radiculitis nerve conduction is violated, and therefore, the regulation that controls the metabolism in tissues. Sharply tissue immunity falls. The skin becomes defenseless against the attack of a permanent external enemy - the herpes virus, which is widespread in the environment.

But if in other parts of the body the “zone of responsibility” of different nerve roots suitable here seems to overlap slightly, the intercostal nerves on the chest wall run strictly along the ribs, in parallel, like stripes in a zebra. Violation of conduction along such a “strip” is not compensated by signals from other nerve roots. That is why a narrow strip of skin along the rib in the event of a nerve pinch is left completely without "instructions" from the central nervous system, and the patient's skin in this area becomes defenseless against the virus.

One of the mysteries of this complication is that herpes zoster only happens once in a lifetime - as a rule, this disease, fortunately, does not recur. Probably, in the blood and tissues in response to a massive attack of the herpes virus, a sufficient amount of antibodies to it is produced and stored.

In addition to impaired ventilation of the lungs due to pain and related problems with the respiratory organs, in addition to infectious viral complications, chest osteochondrosis is also unpleasant by frequent disorders of the emotional sphere. The disease strongly neuroticizes patients.

Constant pains - a consequence of intercostal neuralgia - are such that it begins to appear to the patient: his heart is about to burst. Recall the story of a bank employee who was hospitalized with suspected heart attack.

But how many such patients are there! People for years suffer from incomprehensible pains in the heart, suffer from shortness of breath ("I can not breathe deeply"), stay in a forced posture ("I can not straighten") - and at the same time they do not know what they are suffering from. The man begins to feel that he is incurably sick. And you just need an experienced orthopedic-vertebrologist and a not too long course of treatment for all these symptoms to pass.

Among my regular patients is patient A., 44 years old. In appearance - a handsome tall man, a real Russian hero.

How does lumbosacral osteochondrosis occur?

There are several critical points in the spine, where the S-shaped (when viewed from the side) its bend intersects with a vertical line - a gravity vector.

Every object that a person lifts bent over, when straightening the spine, responds at the lowest point of our $ at a ratio of 1:10. This is done according to the law of leverage: when we bend, the front lever arm (from the vertebral body to the arm) is 9-10 times longer than the shoulder, measured from the vertebral body to the attachment of the extensor muscles of the back.

But on almost all foreign cars seats have a so-called lateral support and lumbar cushion, which corresponds to the physiological lumbar flexure.

Our popular “massage mats” made of wooden beads, placed on the seats, help except by sticking a sweaty shirt to the back, however, this is also not bad. The back will not supercool.

Now there are special massage cushions. They are “turned on” by a special motor, and metered vibrations massage the back well.

In the lumbar spine, a so-called sciatic nerve is approximately as thick as a finger (in Latin Nervus ischiadicus), hence the name of the complications of lumbosacral osteochondrosis - sciatica.

It is already scary to describe the symptoms of this disease, but what is it like to experience? Pinching the door with a finger just hurts. Pinching a nerve of the same thickness between the vertebrae is very painful. The pain spreads to the buttock, under the knee, sometimes to the calf or heel. In advanced cases, part of the nerve fibers may die off - the leg seems to go numb. Symptoms of loss of sensitivity are very persistent, sometimes this numbness persists for a long time.

Among my patients was a man of a rare profession - a diamond cutter. His work required constant intense attention and perseverance, which had a bad effect on his aching back ...

In childhood, he suffered a small degree of scoliosis, and then for a long time the disease did not make itself felt. But as soon as the guy started working, after a few months, pain in the lower back appeared. Unfortunately, he did not turn to the doctor in time. And when his mother called me, the pains worsened to such an extent that the guy could not even walk to the toilet. The slightest attempt to rise from the bed led to the fact that he almost lost consciousness from the pain.

It had to be treated for a long time. Sciatica does not pass quickly ... Unfortunately, the young man had to change the faceting of diamonds to the more prosaic work of the controller at the same company. This work did not involve a long sitting in an uncomfortable position.

The type of patient with sciatica is characteristic: he walks, bending down on the healthy side, in order to maximally open the vertebrae on the diseased part of the body and thereby reduce the compression of the root. Over time, as a result of treatment, the pain disappears or becomes tolerable. If the vertebra is not adjusted, the spinal curvature is retained. In this case, the load on the intervertebral disk, already already damaged, is unevenly distributed and as a result leads to its further destruction.

The most unpleasant complication of lumbosacral osteochondrosis is not even pain. Worst is the instability of the vertebra. The disk no longer fixes the vertebra, under load under the action of gravity, the lumbar region “slips” from the sacrum. Complications of the internal organs come to the fore. The only treatment is operative.

I will not dwell on the surgical treatment of osteochondrosis, I will only say that the absolute indication for surgery is squeezing ponytail syndrome. So anatomists figuratively call a bundle of nerve roots passing in the lower part of the spinal canal, - indeed, it looks like a horse's tail. In this syndrome, loss of function of sensory and motor roots occurs, and, most importantly, dysfunction of the pelvic organs (urogenital sphere). In this case, the patient has paralysis of the lower limbs and their numbness. Perhaps urinary incontinence.

The operation is also assigned when the spinal cord or its vessels are squeezed due to displacement of the vertebrae.

These operations are heavy. Access to the bodies of the lumbar vertebrae is bypassing the abdominal cavity, through a large incision, the vertebrae are fixed to each other. The spinal column is restored, but mobility remains limited ...

Such a sad forecast, but one conclusion - spare your spine! It is very durable and flexible at the same time. Do not treat him barbarously. After all, does it not occur to anyone to hammer in nails with a crystal vase?
